Restrictions:
All UNCE Medical staff is permanently disallowed to enter District Four (4), doing so results in being exiled from the UNCE.
Nurses are advised to listen to doctors, they can't order you around but listening to them is the best way to assure the quality of the medical treatment.
Field patrols are to be held in groups on minimum two UNCE Medical staff members. This is for safety measures and assurance of the medical treatment's quality.
Do's and Dont's:
Do patrol the city in groups of minimum two UNCE medical personell, we encourage being on the lookout for possible injured cases.
Do use your common sense when deciding which citizen obtains priority, a bleeding nose is less worse than a broken leg for example.
Keep your supply of medicine steady, last thing we want is running out of bandages on a field case.
Medical Records are to be kept after each operation, lay the completed records on the metal desk.
Do not hand out medicine to anyone who asks for it, demand a solid reason why they need it.
Do not give friends or family priority on other citizens with worse injuries.
Do not go on field patrols alone, ask a fellow UNCE member to keep you company. There's safety in numbers.
Medical Records:
This is what you will have to write down on a clipboard or paper when you are treating a patient, it's for administrative purposes.
Name of the citizen:
CID:
Injuries/Diseases:
Treatment:
Prescriptions:
Staff who treated the patient:
Brief Report:
These records are to be IC'ly left on the desk and posted in the thread named Medical Records on the forums, this is to prevent loss of medical records during a server crash.
Field Patrols:
Field patrols are performed by all UNCE Medical staff, they consist of minimum two UNCE Medical staff members and are held on a constant basis. These patrols serve as mobile medic squads and provide citizens who are unable to reach the clinic a chance to obtain proper medical treatment.
Applications:
Applications are OOC so you have to apply on forums.
Patients Reception:
If there are large amounts of patients, nurses are to use Triage processes, she will diagnose the patients and see who's going to be prioritized.?
Treatment Procedures:
1. - You will wash your hands under a sterile scrubbing gel and let it dry out or put your hands under alcoholic gel and wipe it out on paper towels, then you will put on a pair of Latex Gloves (Gloves are to prevent infection).
You will then apply the following if necessary: Sterile Gown, Scrubs Hat, Safety-Glasses, Shoe-Covers And Face-Masks.?
1.1- If the patient is coming for a surgery, he will be given a Sterile Drape uncovering the place the surgery will take place.
If the patient is unconscious the Nurse would strip him and do this by force, if the patient is conscious he will do it by himself.
They will then check the respiratory rate, pulse-rate and blood-pressure, the Nurse will put her middle and index fingers on the patient's artery, either neck or wrist, the Nurse needs to count the chest rises for 30 seconds and multiply it by 2. For blood pressure they will will place a blood pressure cuff as they wrap it around the arm slightly above the chest, then you will place the stethoscope under the forearm and pump the blood pressure cuff up to 160. You will hear two different sounds, a stronger and a lighter. At the start of each new sound look at the reading on the cuff to see the numbrs with the first and stronger sound. (Ask the patient if his blood pressure is low/high/normal then deflate the cuff.
Normal pulse for an adult is 60 to 100 per minute. Respiratory for an adult is 12-18 and blood pressure would be high/normal/low as it's nearly impossible to explain it to the patient within a sentence.
1.2- Diagnosing a patient:
1.2.1- Verbally diagnosing a citizen, simply asking him what's wrong.
1.2.2- Inspect the patient, check for wounds etc...
1.2.3- Palpation is when you actually feel the patient to check for his problem (lumps, tumors etc...).
1.2.4- Percussion diagnosis is when you tap a certain part of the body and check for internal bleeding.
You'll have to place your left arm on the area which you need to examine using your index finger of your right hand to tick your left arm's finger.
You will listen to the sound that you've made. The belly area should sound sonor, live should be dull.
If there's internal bleeding, it would usually be dull as well, so if you hear a dull sound you'd need to check it further on.
1.2.5- Stethoscope use, place the stethoscope on the area you wish to examine.
Place the earbuds of the stethoscope into your ear and listen carefully.
-> Internal bleeding - Cripple sound when you move the area gently.
-> Abnormality within the heart - Murmur sound or a gallop sound along with breathing sound.
-> Pleural effusion - Cripple sound within their chest.
-> Pneumonia - Coarsed breathing sound.
-> Cancer - Blood murmur (due to there being neovascularisation).
Wound treatment:
As the wound is uncovered from any fabric, you will need to sterilize it.
2.1- If it has any fragments on it, you will pour Saline Solution over it to clean it. You will then take any big fragments with a tweezers around or on the wound.
Wash the wound with hydrogen peroxide and wait five minutes.
Wash the wound with saline solution again.
2.2- In case of treating an open wound treatment, sterilize (see 2.1), then proceed to clot it.
Take a sterile Gauze Pad and then place the pads on the open wound with a pair of tweezers.
Apply Trauma Tape if necessary.
2.2.1- If a small wound needs to be closed you will need to apply steri-strips, medi-gel etc.
2.2.2- Big wounds that require stitching would be sutured with a suturing kit.
You will take your suturing needle and thread as you start suturing it with a pair of forceps, stitching the wound all the way as you make knots and then you'd cut the needle, bandaging the stitch for one day.
Fractures:
There are a few kinds of fractures, as a Clinician in hl2rp you'd only be able to help a few.
Any fracture that may get your patient disabled shall be contacted to the UCP for authorization beforehand.
There are two kinds of fractures: Simple and Compound:
Simple = The skin is intact and the bone is underneath the skin.
Compound = Bone is broken through the skin and exposed.
Surgical repair and Non-surgical repair.
3.1- Non Surgical Repair, inspect the injury and check if it's a fracture, if possible due not having an x-ray, administer local anesthetic, re-position the bone with force, lining it correctly.
Mold a cast or a common splinting device to prevent further injury.
3.2- Surgical Repair, place patient under general anesthetic, re-position the bone or fracture. Use surgical screws or bolts if needed and insert the bone in place. A bone saw can be used if needed.
Mold a cast or a common splinting device to prevent further injury.
Injections:
4.1- Intravenous Therapy is a way to give liquid injections directly into the vein.
You will need to insert an Intravenous needle, usually in the top of the hand or lower inside the arm. You take an IV needle and catheter, insert the needle until it is inside the vein, push forward the catheter into the vein then remove the IV needle. You'll need to tape the catheter then to secure it. Catheters would let you inject the patient easily, the top port is where you'd put the IV tube then the syringe.
4.2- Intranasal - Used for the nasal delivery of drug/s by the use of nasal sprays.
4.3- Intramuscular (IM) - The injection of a substance directly into the muscle
4.4- Tropical - The cream/gel/lotion is applied directly to the body surfaces such as the skin or mucous membranes.
4.5- Oral - The giving of pills/drink through the mouth.
Anesthetics:
5.1- Local Anesthetics are used for minor injuries (most common).
It usually keeps the patient conscious.
5.2- General Anesthetics is used for Major injuries, it makes the person go unconscious.
General anesthetic drug we have permission to use is Methohexital due us not being able to use gases.
Medicine Administration:
Cardiac medications - Adrenaline, via IV. Raises blood pressure.
Verapamil - Lowers blood pressure.
Atropine - Stimulants pulse.
Anaesthetics - Lydocaine HCl, Propofol, Methohexital.
Paramecatol is a minor pain relief.
Morphine is major relief via IV.
Topicort is for burns and skin pain relief.
